Application for Retirement
You should obtain this form from your local personnel or business office sometime in mid-April of the year of your planned retirement. Complete Section I and return the form to your personnel or business office where Section II will be completed. Your employer will forward the completed application to our office. Our staff will review your application for eligibility and mail you: (a) An information memorandum; (b) A final computation of your retirement benefit; (c) A final selection of benefit form; and (d) A direct deposit authorization form. The New Mexico Retiree Health Care Authority will automatically mail you a retirement packet.
